buss cables fall out of guide when bow is fired

what would the reason for the buss cables on a twin cam compound bow to fall out of the cable slide when fired? I have a slide that sits on the outside of the 3/8" rod and when the bow is fired, the front mount buss cable falls out of its seat, do you have any suggestions.
